The immune system was the week’s biggest biotech story - as both the disease and the cure.

The clearest result came in Guillain-Barre syndrome, a sudden autoimmune condition where, after an ordinary infection like food poisoning or flu, the body starts attacking its own nerves [1]. A late-stage trial of a drug called tanruprubart gave 241 recently diagnosed patients a single infusion or a placebo. Those on the drug recovered sooner and more fully on every measure [1]. It blocks a blood protein called C1q, switching off the branch of the immune system misdirected onto the nerves [1]. It would be the first targeted treatment for the disease, and could be approved next year [1].

It was one of several attempts this week to turn a misfiring immune system down. In Sheffield, a man with multiple sclerosis - where the immune system strips the coating off nerves - became the seventh patient in a global CAR-T trial [2]. CAR-T is a tool first built to fight cancer, now repointed to reset the immune attack [2]. Pfizer said an oral drug restored skin pigment in late-stage vitiligo trials, a condition where the immune system destroys the cells that colour the skin [3]. And Vertex paid AbCellera $28 million upfront for a new autoimmune drug, part of a wave of money chasing the same target [4].

The other half of the week ran the opposite way - the immune system failing to see an enemy disguised as self. A Merseyside woman became the first in the world to get a cancer vaccine made from her own tumour [5]. It is meant to help her immune system recognise a cancer that had learned to hide from it [5]. An FDA panel voted in favour of Replimune’s melanoma drug, which works by making tumours visible to the body’s defences [6]. And J&J agreed to pay up to $2.6 billion for Sail, a maker of in vivo CAR-T [7].

Why your immune system's hardest job is knowing what not to attack

Your body's defence carries no map of you - it learns restraint, imperfectly, and most immune disease is that learning going wrong.

How it works

  1. Your immune system must attack invaders and spare your own cells.
  2. It learns which cells are 'self' - mostly by killing off, early on, any immune cell that reacts to you.
  3. That learned restraint is called tolerance, and it is fragile.
  4. Break it one way and the system attacks your own body - autoimmunity.
  5. The enemy exploits the same rule, disguising itself as self to escape.
  6. Every disease here is one misjudgment of self versus non-self.

The twist

Your immune system holds no list of your body's parts - it learns, early and imperfectly, what not to attack. So tolerance is not knowledge but a fragile act of restraint, and both the disease and the cure live in whether it holds.

The defender turns on the defended

A person eats something bad, fights off the bug, and recovers. Then, days later, their legs stop working. The immune system that just saved them has started attacking their own nerves.

That is Guillain-Barre syndrome. It is strange in a way worth sitting with. The thing built to protect you is now the thing harming you - and it is using the exact same weapons it used on the infection. To understand how, you have to understand what the immune system actually knows about you - which is almost nothing.

The system has no list

Your immune system does not carry a blueprint of your body. It could not, really. Your cells change by the hour, and no fixed list could keep up.

So it learns instead, early and by a brutal method. Young immune cells are tested against your own tissues while they mature. Any that react too strongly to your own cells are destroyed on the spot, before they are ever released. What graduates is a fighting force that, by construction, mostly ignores you.

That learned habit of not-attacking is called tolerance. It is the whole foundation everything else stands on.

Tolerance is restraint, not knowledge

Here is the part that reframes every immune disease. Tolerance is not the immune system knowing which cells are yours. It is the fact that the cells which would have attacked you were removed, and that a few backup brakes hold the rest in check.

There is no positive knowledge of self anywhere in the system. There is only the absence of the attackers, kept out by fragile safeguards. Break a safeguard and the weapon that guards you swings inward. Guillain-Barre, multiple sclerosis, vitiligo - each is a different one of those safeguards failing, in a different tissue. In one, nerves. In the next, the coating around nerves. In the next, the cells that colour your skin.

The enemy reads the same rule

Now the flip side, and it follows from the same fact. Because tolerance works by not attacking whatever looks like your own cells, anything that can pass as your own cells is safe.

A tumour is your own cells gone wrong. It already half-belongs to you. The tumours that survive are the ones that also wear extra signals that say, in effect, do not attack me. The immune system, trained its whole life to spare self, obeys. Some infections pull the same trick.

The mercy the system shows toward you is exactly the gap the enemy climbs through. That is why the newer cancer drugs work the way they do. One is a vaccine made from a patient’s own tumour. Another is a virus that lights the tumour up. Both try to re-teach the system to see what it had been trained to spare.

One dial, two ways to fall off it

Put the two halves together and they are not opposite problems. They are the same judgment failing in opposite directions.

Turn the immune system up too hard and it attacks your own body. Turn it down and disguised enemies walk past. Every drug in the news this week was nudging the same dial. The Guillain-Barre drug turns it down, off the nerves. The cancer vaccine turns it up, onto the tumour. And there is no neutral setting in the middle: calm the attack on your own tissue and you also lower the guard against real infection. The machinery that protects and the machinery that harms are one machine.

The calculation you are running right now

This is not a disease of the unlucky few. Everyone carries the same imperfect training, running the same guess every second.

Allergy is that guess held too tight - a full alarm raised over pollen or a peanut that was never a threat. A rejected transplant is the guess refusing a gift, attacking a donated organ as foreign. A pregnancy is the strangest case of all: the one time the body deliberately stands down and tolerates something half-foreign growing inside it.

You are not watching this from outside. You are the fragile calculation. It holds, quietly, because most of the cells that would have turned on you were culled before you ever knew they existed. And the line between you and the world, which feels so obvious, is something your body has been guessing at all along.
